Patient Forms
To help save you from needing to arrive early to fill out paperwork in-office, we offer online new patient paperwork that you can access by clicking on the link below. Simply fill out the information and submit it using our secure portal. Our team will receive it immediately and input it into our system to make your first appointment smooth and hassle-free!
Dental Insurance
Our office is proud to now be in-network with most dental insurance plans since becoming a Grand Dental Partner! So, if you’ve come to us in the past and haven’t been able to maximize your benefits to the fullest, you can now! Give us a call to learn more about our in-network dental insurance plans and how our team can help you save by filing claims on your behalf.
Grand Advantage Plan
Our Grand Advantage Plan is a great alternative for our uninsured patients. For a low annual fee, you’ll receive access to fully covered routine treatments as well as a 20% discount on any additional services you receive.
Financing Available
If you have a high out-of-pocket cost that you’re unable to pay all at once, we make it easy to break it up into smaller installments using CareCredit or Alphaeon. Upon approval, you’ll be able to sign up for a payment plan that allows you to split up the cost of your treatment into smaller, easy-to-manage monthly amounts. Military Discount
Veterans of the United States Military that do not have any other source of dental insurance or dental discount plans may participate in the Grand Advantage Plan (GAP) at no cost to join. GAP is a membership plan that was created by Grand Dental to provide affordable dental care for individuals who may require support to access the oral healthcare they need and deserve. GAP provides a 20% discount on all services offered at any Grand Dental Group location for a $49 annual fee to join. Veterans, who fit the above criteria, and their immediate family members are invited to join our GAP program for free. Grand Dental Group will pay the annual membership fees.
